No. Foundation Health MD is a consultative lifestyle medicine practice and is not intended to replace your primary-care clinician.

You should continue seeing your established medical team for routine primary care, preventive screenings, ongoing medication management, acute illness, and urgent or emergency needs. Foundation Health MD focuses on a deeper assessment of lifestyle, health risks, goals, and opportunities for meaningful change.

The Initial Comprehensive Assessment is a 60-minute physician consultation.

Before the visit, you will complete intake forms and may upload relevant medical records and laboratory results. During the appointment, Dr. Schulist will review your:

• Medical history

• Current concerns

• Lifestyle patterns

• Health risks

• Available laboratory information

• Priorities and goals

The visit will concentrate on the areas most likely to make a meaningful difference. Because an entire medical history cannot be solved in one hour, one or two concerns may be explored in greater depth.

You will receive personalized recommendations and a written action plan following the assessment.

This service may be helpful for adults who want a physician-led review of their health, lifestyle, risks, and goals and who are ready to consider practical, sustainable changes.

It may be particularly useful for people seeking guidance related to metabolic health, healthy aging, disease prevention, physical function, sleep, nutrition, stress, or overall well-being.

The fee includes:

• The 60-minute physician consultation

• Review of the information submitted before the visit

• Review of relevant available laboratory results

• Personalized recommendations

• A written action plan

Laboratory testing, follow-up visits, prescriptions, and services provided by other clinicians or facilities are not included.

No. Laboratory testing is not included in the consultation fee.

Relevant existing results may be reviewed when submitted before the appointment. If additional testing appears useful, Dr. Schulist may recommend that you discuss it with your primary-care clinician or arrange it separately.

Foundation Health MD does not currently prescribe or directly change medications.

Your medication regimen may be reviewed during the assessment, and Dr. Schulist may identify questions or options to discuss with the clinician who manages your prescriptions. Medication changes should be made in coordination with that prescribing clinician.

Yes. Visits may be provided by secure telehealth when a virtual visit is clinically appropriate.

Some concerns cannot be adequately evaluated remotely and may require examination, testing, or care from a local clinician. Telehealth is not appropriate for emergencies.

You must be physically located in a state where Dr. Schulist is licensed and Foundation Health MD is currently accepting telehealth patients at the time of your appointment.

Your physical location will be confirmed at the beginning of the visit. Telehealth is legally considered to occur where the patient is located, not where the physician is sitting.

Current telehealth availability: North Carolina, Wisconsin, Illinois, Pennsylvania, Florida and Georgia.

Yes. Foundation Health MD is located in Pisgah Forest, North Carolina, and welcomes patients from Asheville, Brevard, Hendersonville, and surrounding communities. In-person consultations are available in Pisgah Forest, with telehealth also available to patients throughout North Carolina when appropriate.

No. Foundation Health MD is a self-pay practice and does not submit claims to commercial health insurance plans.

Fees are disclosed before scheduling and payment is made directly to Foundation Health MD. Patients may ask their insurance plan whether any out-of-network reimbursement is available, but Foundation Health MD cannot guarantee coverage or reimbursement.

If you are enrolled in Medicare, please contact Foundation Health MD before scheduling so we can confirm whether the consultation can be provided on a private-pay basis.

Yes. Foundation Health MD is designed to complement, not replace, ongoing primary care.

Your primary-care clinician remains responsible for routine medical care, preventive services, acute concerns, medication management, and coordination with specialists.

Please complete all requested intake forms and upload relevant medical records and laboratory results before the stated deadline.

Foundation Health MD does not provide emergency care, urgent care, after-hours coverage, or continuous medical monitoring.

Foundation Health MD PLLC is an independent, physician-owned medical practice that provides consultations to eligible Skyterra guests.

Foundation Health MD and Skyterra are separate organizations. Medical decisions, informed consent, medical records, payment, and clinical communication related to your consultation are handled by Foundation Health MD. Skyterra does not direct Dr. Schulist’s medical judgment or control the care provided.

No. You do not need to be a Skyterra guest to schedule with Foundation Health MD. Services are available to other eligible patients, subject to location, licensure, and scheduling availability.
